Weekly Hero Awards recognizes excellence and invites constructive concerns to improve care. Keep submissions factual, respectful, and privacy‑safe. We do not publish identifiable health information.

• Two streams: Applause (positive recognition) and Concern (needs follow‑up).
• Concerns are reviewed privately; we may contact the community for resolution steps.
• No PHI/PII about residents without consent. Avoid diagnoses and sensitive details.
• We reserve the right to edit for clarity and privacy.
• Media release is required for public features.
